  • Need

For many suffering with life-limiting conditions hospices can be very daunting and some patients hide their pain unnecessarily. We believe patients should be able to remain in their preferred place of care which is why our Hospice at Home and night-sitting services are vital.

Providing care at home can be very demanding, both emotionally and physically. Many carers are elderly and some experience their own physical disabilities which affect the level of care they are able to give the patient.

We believe patients should be able to remain in their preferred place of care which is why our Hospice at Home and night-sitting services are vital. Our Hospice at Home service delivers the specialist care that we provide at the Hospice directly into the heart of the communities of Hastings and Rother.

We provide complete peace of mind around the clock in the patient's home, while offering a tailored service that best suits individual needs and supporting the wider family and loved ones.

  • Aims

    Aim 1

To offer adults with life-limiting conditions the benefit of receiving palliative care at home

» We will offer free of charge Hospice at Home visits, providing around the clock pain relief and emotional and physical support to the patient.
» We will give patients the choice to remain at home for palliative and end of life care. The opportunity to remain in familiar surroundings is vital.

    What success will look like

Success will be enabling patients the opportunity to have greater input into their own care and enabling them to remain at home, surrounded by loved ones and home comforts.

    Aim 2

To support the carer and the patient's loved ones during difficult times.

» Support from Hospice at Home will provide the patient's carer the opportunity to rest safe in the knowledge that the patient is being cared for.
» Our team will allow the carer to spend more quality time with the patient during a difficult and challenging period.
» Hospice at Home services will enable the patient's loved ones to continue with daily responsibilities without worry.

    What success will look like

Success will be seeing carers and loved ones being more relaxed and able to spend a greater amount of quality time with the patient, creating lasting memories together.

  • Impact

Hospice at Home visits not only provide pain relief and comfort to patients who wish to remain at home, but also provide support, confidence and independence to the entire family. The service enables patients to have a say in their own care requirements and even the option to remain at home in familiar surrounding in their final days.

We will work closely with beneficiaries to monitor the number of people helped as well as gathering feedback where suitable.

There is a risk that the service will not be used to full capacity. However, we are very confident that this service is, always will be, in very high demand.

We will continue to liaise with healthcare professionals to raise awareness of the Hospice and Home service and to encourage referrals for those who would prefer to receive palliative care in their own homes. Not only will this benefit the patients and their carers, but also reduce demand on public health services.

We will provide a regular update to donors to share news of the service and how their donations have helped those in need.

We will also feature an article in our newsletter reporting on the success of the service and how The Big Give Christmas Challenge supported this.

  • Background

We offer our services to the communities of Hastings and Rother. Our catchment's most populated area, Hastings & St Leonards, is the most deprived district in East Sussex and the seven most deprived wards in East Sussex fall into this district.

The proportion of elderly residents on low incomes is higher here than in any other town in the South East and patterns of terminal illness, including the health consequences associated with smoking and alcohol, reflect these social conditions.

Hospice at Home services will be offered free of charge to adults with life limiting conditions throughout the communities of Hastings and Rother. Furthermore, the services supports the wider family network with physical and emotional support during challenging times.

Our palliative care and end-of-life services are available to anyone with a progressive life-limiting illness such as cancer, heart failure and respiratory and neurological conditions.
