Community Link Worker

Older people fear social isolation more than death. Over the decades we have helped older people to remain active in their families and their communities. Like most of us, a time comes when they need more support. The Community Link Worker's role is to help people in later life access our services.

  • Need


    Older people fear social isolation and loss of independence more than death. As people age, most become less independent and more isolated. We have resources to help older people improve the quality of their lives but a gulf opens up between them and us to do with transport, mobility problems, fear, anxiety, loss of confidence and an unwillingness to ask for support.


    The Community Link Worker's role will be to provide outreach for Hoxton Health. They will keep an eye on people who have been users of our service and whose use changes and keep in touch with them to make sure we know if they need additional help.

    They will also make links with nursing and care homes as well as the discharge staff in our local hospitals to ensure that residents and patients know that our services are available to them and how to access them.

    Aim 1

    To ensure that current service users don't disappear from sight unnoticed.


    » Regularly checking the database to ensure service users don't change their use of our services unnoticed.
    » Following up on anyone to ensure that they have the support they need.

    What success will look like

    By being in touch with people who cancel or do not attend their appointments and finding out whether they want further support or not.

    Aim 2

    To provide outreach to care and nursing homes so residents have to option of accessing our services.


    » Visiting care and nursing homes, giving presentations to staff and service users about Hoxton Health and what we can do for them.

    What success will look like

    By making links with local care homes that might lead to outreach clinics, individual home visits or on occasion residents coming to our clinic.

    Aim 3

    Liaising with Discharge nurses as local hospitals so that patients have access to our services.


    » Ensuring that Discharge Teams have plenty of information to give patients about us and meeting patients before discharge if they want our services.

    What success will look like

    By discharged patients being referred from hospital.

  • Impact


    The success of this project will allow Hoxton Health to continue to support people in very late life and for our services to enable people to improve the quality of their lives and stay as active and interested in their communities as possible as they age.


    The person undertaking this role will be working in an outreach role and will often be working on their own in the community. Good recruitment for this role is therefore vitally important to ensure that we have someone who is conscientious, caring and confident.


    We will provide a newsletter for donors outlining output and outcomes as well as case studies that illustrate that we are reaching our goals. As they will also be joining our broader family they will receive our Hoxton Health newsletters.

  • Background


    We are based in Hoxton, one of the poorest wards in the UK. Hoxton is now synonymous with Hipsters and is a very up and coming area but rather than improve the lives of older people it is damaging their lives as old eateries and cafes are closed to make way for more trendy offerings.

    We work across the whole of Hackney and The City of London but we target specifically those who can least afford the services we offer on the open market.

  • Why Us?

    Why Us?

    We have worked in this geographical area undertaking this work with older people for 29 years.

    We were pioneers when the project started and continue to pioneer which has from time to time brought us close the the financial edge. It is more important for us to provide the services that people want than those that will keep us most financially secure.

    We are pursuing this path because we have seen the need and we want to help more people but at the moment we don't have resources to reach out.
