Green Moments aims to provide regular opportunities for people with dementia and their carers to: access and enjoy meaningful outdoor activities,to be able to connect with the natural environment, work together, support one another, exercise and feel included in their local community.

It is important for people living with dementia to continue living active and meaningful lives. Research has found that greater cognitive impairment is associated with reduced adherence to exercise. Green Moments (GM) is one way of reaching and engaging with vulnerable people to motivate them to get outdoors & be active. Dementia can be an isolating condition making people feel lonely & cut off from their loved ones & the wider community. GM helps nurture relationships & engage in the community
Projects offered at Green Moments (GM) are inclusive for all abilities, tasks are adaptable & vary to meet the needs of the individual, to keep it fun and accessible. GM also focuses on encouraging independence, engagement and giving people a sense of purpose. GM also encourages communication, social interactions and new friendships. All are crucial in giving people the confidence that they are supported in the community and that they are not alone.

Activities» Gardening as a physical activity has been shown to be helpful in the treatment of anxiety, depression and dementia.
What success will look like
Questionnaire and feedback relating to: feeling of independence and physical abilities, enjoyment, confidence, ability to cope with stress
To reduce loneliness & social isolation
Activities» Green Moments provides opportunities for people to meet others in similar situation as themselves.
» They are able to be in a non-judgemental environment that encourages people to become more engaged in their local community.

Asking if questions regarding: friendships and if Green Moments provides an alternative outdoor activity to other groups? Coordinator observations of group dynamics and friendships

What success will look like
This will focus on Green Moments providing a space for people with a dementia diagnosis AND their carers to develop their relationships and gain support from others in the group
Maintain health & wellbeing- weight management, mental health, reduced: risk of ill health, falls; independence, improved mobility, better quality of life
Reduce loneliness & isolation- Remain engaged & active, better relationships with loved ones, community education & awareness, feeling valued/sense of purpose
Increased support network- reduced burden, carers better able to cope with stress & anxiety, pro-active support, carers able to care for longer, reduce pressures on health & social care
Risk would be if the director of Roots to Growth could no longer run Green Moments. Other local options have been considered.
Risk of low attendee numbers,if after one year of funding and unsuccessful marketing the project would either change and move to another venue or end.

Green Moments is located in Washington, West Sussex. W.Sussex has a higher percentage of adults over the age of 65 than the England average. It is estimated that 1 in 3 people over the age of 65 will develop dementia. With a growing ageing population these numbers are expected to rise. Around 1/4 of the population live in rural areas. With the cost of living in rural areas being significantly higher & services harder to access, there is evidence that deprivation is getting worse in Sussex.
Green Moments is committed to provide education, advice and support to people living memory problems and their carers. Although we offer support to anyone with memory problems, which may have been caused by a number of causes such as: stroke, Parkinson’s or dementia, we mostly support people living with dementia and their loved ones. We utilise a number of methods to inform the community of what services we offer, for example: GP referrals, Alzheimer's Dementia Advisers, posters, online, etc
Know Dementia a local charity, run by volunteers comprising of family carers & qualified professionals who live and work with people with a dementia. All the volunteers, employees and trustees are passionate, have understanding & experience with dementia within the local community. Director of the community garden says 'we are trying to achieve a new community where people are welcome & accepted whoever they are' & 'this is space that enhances how we we can engage people' in a therapeutic space
