    Need

Referrals to our home care service are increasing. More and more people are expressing their desire to receive care in the comfort of their own homes, in familiar surroundings and amongst their favourite belongings. We are also seeing an increase of people with more complex illnesses being referred for support.

    Solution

“At Compton Hospice we recognise that people aren’t just sick between the hours of 9am to 5pm, Monday to Friday, and so we have expanded the support we offer in order to reach more people with incurable illnesses across Wolverhampton and the Black Country. Compton Hospice is the only adult hospice in the Midlands to offer these extended hours of care.

Community Nurses regularly visit patients’ to discuss and monitor symptom management, pain control, health, medication and emotional support.

To meet increased demand for care in the home for patients with incurable illnesses.

  • Impact

    Impact

The expansion of our Community Nursing Team and hours of operation means we will now provide support between the hours of 8am and 9.30pm, 7 days a week, due an increased demand for care in the home.

Referrals to our home care service are increasing. More and more people are expressing their desire to receive care in the comfort of their own homes and we expect that this will continue to increase.

We will demonstrate this projects success with patient and family feedback.

    Risk

The risks affecting this project are financial. We are a charity and rely on public donations to continue to provide a high level of care to people with incurable illnesses and their families.

Our five year strategy has identified growth areas in our charity shops and fundraising arms to enable us to continue to provide palliative care in the home and continue to do so into the future.

    Reporting

By offering regular feedback from patients and their families as well as issuing figures on the number of patients seen in their own homes.
