Hospice at Home

Research shows, 75% of us would prefer to spend their last days at home, rather than in hospital or a hospice. The Isabel Hospice at Home specialist service enables families to stay together at this emotional time and carers are given confidence to care for their loved one at home with support.

    Need

    Isabel Hospice is trying to reduce unnecessary hospital admissions for those with life-limiting illnesses, to provide end of life care at home for those who need it and to prevent carer burnout or breakdown. We also know that it can take up to 2 weeks for continuing health care packages to be implemented after a hospital discharge. This is distressing and worrying time for patients and family members.

    Solution

    Families particularly appreciate the understanding shown by the Palliative Care Support Workers when as non-professionals they are faced with caring tasks they have never attempted. The PCSWs take the time to explain and demonstrate what they are doing so that the family members can take these tasks on themselves. The confidence they gain helps them to cope and helps to avoid emergency hospital admissions which are unsettling and unnecessarily upsetting to the patient and carers.
