We want to give our patients choices - do they want to be cared for in the hospice or in their own home? To be able to offer this choice we are extending our Hospice at Home service to the communities of Hyndburn and the Ribble Valley and will continue to provide the service in Blackburn with Darwen

To provide direct care and support to patients and their families in the last 12 months of their life.
Make living with dying better for patients, families and local communities
Be the patient and families 'Sat Nav' throughout their journey
The service will be responsive to patients needs
The service will prevent avoidable hospital admission
It will prevent crisis occurring by the creation of early referrals into the service and providing responsive intervention to meet the patients changing needs
Provide direct care and support to patients and families
Activities» Provide by a multidisciplinary first contact team with a wide area of expertise
What success will look like
Success will be demonstrated by the number of patients that receive our care that will meet their changing needs.
The project will help to prevent unavoidable hospital admissions and use specialist skills only where appropriate.
Funding is required to ensure this service can be provided. Funding is already being sourced that will secure the project over a 18 month to 2 year timescale
Donors will receive a bi-annual newsletter giving updates on the difference their support is making by helping us to care for the patients in our comunity
