EquipTransition Unit For Life-Limited Young People
We are going to create a special unit for life-limited young people with health issues as diverse as cancer and muscular dystrophy, to enable them to have respite care and pain management, as well as a dedicated hospice space that's equipped for their age group, with the facilities they need.

There is a gap in hospice provision for teenagers and young adults in the north-east, who are too old for children's services, but too young for adult services which are aimed towards much older people. This age group of life-limited young people require appropriate facilities to fulfill their care needs and to be given assistance to be as independent as possible, to be involved in their own care choices and to be able to make decisions about their end of life care.
We will build and equip a unit which has facilities which are appropriate for this age group, including a kitchen for independent food preparation, suitable IT and entertainment facilities, and create a lounge/social area where young people can socialise and relax. We will accept referrals for new patients from outside of our locality, to enable as many young people as possible to access respite care and specialist nursing when required.
To equip our new transition unit with facilities appropriate for patients aged between 13 and 25.

Our transition unit will work with young people to help them move from children's services to adults, by working closely with appropriate outside agencies and professionals. The unit which we wish to equip will be used to teach independent living skills such as budgeting and cooking, something which is not possible in our children's hospice. Success would be having 25 young people/families accessing services regularly within 12 months and who respond positively to an annual survey.
Delay in fitting. We have addressed this risk by allowing a larger time-frame for works to be completed, in case of contractor delays or unforeseen circumstances.
Funders of the project will receive a copy of our regular newsletter which will highlight all areas of our work, including our new unit. Our website will also be updated as the works progress.

The unit will be based beside Butterwick House Children's Hospice in Stockton On Tees.
Life-limited young people and their families from across north North Yorkshire, Teesside, County Durham and Wearside.
We have nearly 30 year's experience in delivering adult hospice services and 15 year's experience in delivering children's hospice care. We are well-known and trusted by families across the region, and have already identified 25 young people who have left children's services who will benefit from this facility.

Alison is the Children's Clinical Lead, she will ensure that all items are placed appropriately so the young people have maximum access.
