Give a Gallon

Our St Clare Hospice Specialist Palliative Care Community Nurses visit patients and families in their own homes to provide support, information and advice. The team work alongside GP's and District Nurses to plan, deliver and respond to the changing needs of families at home.

  • Need


    Patients and families dealing with a terminal illness can feel very isolated and alone. They are unsure about the future and about the treatment and care options available to them.


    Our nurses make home visits and liaise with patients and families at every stage of their journey.

    The aim of this project is to support patients and families through their diagnosis and symptoms.

  • Impact


    Referrals to the community team will increase, patients and families will feel more secure and supported.


    We have a risk management group which meets quarterly and this monitors, records and manages risk for all hospice services on an operational basis.


    Funders of this project will receive a quarterly report on the project .
