  • Need

    Need

THis programme is aimed at equipping patients and their families with the information and skills which will enable them to stay at home as much as possible during their ilness and avoid unnecessary hospital admissions.

    Solution

BY offering tangible skills and emotional support in a group of like minded people living with the same condition we hope to help patients to plan for their own pathway to well-being

  • Aims

    Aim 1

To help people live independently and positively for as long as possible

    Activities

» By providing a programme of services and supports

Success will be measured by talking to patients and their families.

    Aim 2

TO help people avoid unwanted admissions to hospital through using their own skills and knoweldge.

    Activities

» as above. By the end of the programme the client should have been helped to write a crisis plan to guide and support them through crisis situations.

Success will be measured by talking to the specialist staff working with patients and counting the number of emergency admissions to hospital.

  • Impact

    Impact

It is important to harlington Hospice to become a source of support for people with non-malignant illness in the same way as it currently supports many people with malignant illness.

    Risk

There is a risk that people with these conditions will not wish to attend the hospice as they do not see it as being a service for them. We are working closely with specialist nurses to ensure they are fully supportive of the work we are doing.

    Reporting

WE will report on the work we do and the evaluation of this via our webpage and our monthly newsletter.

  • Background

    Location

The Harlington Hospice Reg Hopkins Centre was completed in December 2011 and provides a serene venue for patient care. It is situated amongst our hospice gardens and offers both working space and quiet places in which to think.

    Beneficiaries

The hospice offeres care free of charge to referred residents with a Hillingdon GP. Initially this programme will be offered to patients with Heart failure but after evaluation it is intended to offer it to people with a broad range of diagnoses including lung disease, cardiac or neurological conditions.

  • Why Us?

    Why Us?

Obviously Harlington Hospice is already providing care at the end of life for many patients. Through this work we have become aware that there are many other people who might benefit from hospice support but who would never consider themselves in this way. By expanding usage of the hospice to non cancer sufferers we believe we make the hospice even more relevant to our local community.
