Care Team - Family Support Worker

We have grown and now provide care and support to 326 children with life shortening conditions and their families, including siblings, across West Yorkshire. Due to this growth we are now in need of a Family Support Worker to continue our support and enable us to reach out to entire families.

This project is ongoing as we continue to reach out to more and more families who need our support.

    As an organisation that is widely known to be pioneering in its field, we have huge ambitions to grow, so that we can deliver our care and support more children living with life shortening conditions across the region.

    Our mission is to say “yes” to children and families. It is not for them to fit to our service, but for us to deliver what they need in the way that works best for them.

    Forget Me Not Children’s Hospice is about making precious memories for our children and families.

    Solution

    Forget Me Not Children’s Hospice isn't your average hospice! A large proportion of our support is delivered in the homes of our children and families, through our “Hospice at Home” service. Our highly qualified and dedicated Care Team take as many aspects of our care and support directly to our families so that they can remain in their own comfortable and familiar surroundings, and so that the lives of siblings and other family members are kept as normal as possible.

    In the long term we hope to be able to offer every family who has a child with a life shortening condition across West Yorkshire the opportunity to receive specialist care, tailored around their own specific wants and needs.

    We are constantly growing as a charity and have a financial plan in place to help us as we continue to grow, coupled with our experienced team of fundraisers, our ten shops and our fantastic volunteer base

    Reporting

    Donors can be kept up to date in a number of ways, from letters to magazines. We also have a presence on social media (Facebook, Twitter and YouTube) offering donors the opportunity to keep up to date with the latest news from the charity

    Location

    Our hospice, Russell House, is located in Huddersfield. The care we provide can also be offered in families own homes across West Yorkshire.

    Beneficiaries

    Our catchment area is Calderdale, Kirklees, Bradford and Wakefield. Children or young person with a life shortening condition and their family members can access our services.

    Over the past year we have seen the number of families we support continue to increase, in the summer of 2013 we were caring for 50 families and now we support 326. We have the staff and the infrastructure in place to successfully continue to grow and support even more families who have children with life shortening conditions who could need our help
